Contact
This page is for general questions about the practice. New patients begin with the Get Started form; pharmacies and other providers should call.
Becoming a New Patient
New patients start by completing the Get Started form. Dr. Narlesky personally reviews every submission and reaches out directly to those who are a good fit for the practice.
New patient appointments are available on a rolling basis.
General Questions About the Practice
For questions about insurance, scope of practice, or how virtual visits work, use the general inquiry form below. Submissions are reviewed periodically and are not monitored continuously.
This form is not a clinical channel. Please do not include symptoms, medication details, or other medical information. Anyone seeking care should complete the Get Started form instead — submitting the inquiry form does not begin the intake process and does not establish a physician-patient relationship.
Pharmacies and Other Providers
Pharmacies, referring clinicians, and other providers may call 603-722-0409. This line is for pharmacy and provider calls only. Prospective patients will not reach the practice by phone and should use the Get Started form above.
This site is not a crisis resource. Call or text 988 for crisis support. If you or someone else is in immediate danger or is having a medical emergency, call 911 or go to the nearest emergency room.
Dr. Narlesky treats adults 18 and older who are physically located in New Hampshire at the time of their appointment. The practice does not provide in-person care, inpatient or crisis services, forensic or disability evaluations, standalone psychotherapy, or treatment for patients whose primary need is benzodiazepine prescribing. Visiting this site or submitting a form does not establish a physician-patient relationship.
